For your convenience, here is a link to the current permission slip, for you to print out, that we are using for our ACC outings. If you would like to have this permission slip to be used for the whole school year...simply write "Annual Permission" across the top. Then have your youth turn it back into Teresa Goldstein. There are also hard copies of this same form in the kiosk located in the foyer of the church building.
